   A Living Will makes known your desire regarding life support or specifically, the removal from life support and life prolonging procedures.

   The Healthcare Surrogate allows a trusted individual, or multiple individuals, to make medical decisions on your behalf if you are ever unable to do so.

This Instrument outlines your final wishes including:

  • Organ donation preference.

  • Autopsy, if relevant.

  • Comfort care including pain management or hospice.

  • Final blessing prior to passing.

  • Cremation or burial.

  • Preference for wake or funeral and additional final arrangements.
